[#20] Add Task 5 for question_name API parameter

- Add task-5-question-name-api-parameter.md with implementation details
- Add CLAUDE.md for developer context when using Claude Code
- Update README with Task 5 in execution order and task list
- Update GitHub issue with Task 5
