strict_loading_by_default on User and Image (#4510)

Closes the risky tier from #4510. Both models surface N+1s on
the associations every show / index path already eager-loads
(via Observation.matrix_box_includes for the obs flow, plus the
per-controller `show_includes` trees built in #4508 and the
following sweep).

Test plan (all green):
- bin/rails test test/controllers/ — 1992 tests
- bin/rails test test/models/ test/integration/ — 1002 tests
- PARALLEL_WORKERS=1 bin/rails test test/system/ — 60 tests, 1020
  assertions
