fix(knowledgepolicy): score reverse decay before visibility suppression

- pin reverse/inverted decay fresh-access scores to scoreFloor instead of raw zero
- run lightweight decay scoring before honoring stale VisibilitySuppressed flags
- keep explicit suppression as fallback when no scorer or decay binding applies
- verify ON ACCESS only fires after rows pass the scoring gate and materialize
- add regression coverage for reverse scoreFloor starts and stale suppression recovery
