MCP OAuth: move /authorize and /token to the site root (#1741)

Claude.ai's MCP connector with manual client_id/client_secret doesn't read
.well-known metadata; it hard-codes /authorize and /token at the issuer
root. Move our endpoints there to match what Claude actually requests, and
update the authorization-server metadata to advertise the same paths.
