Fix exception traceback retention after swallowed errors (#1316)

Avoid preserving normal-mode tracebacks when OmegaConf raises formatted exceptions, and avoid reattaching active tracebacks when initialized OmegaConf exceptions are re-raised through attribute access or merge wrappers.

Add GC-disabled regression coverage for missing structured attributes, unsupported object creation, and merge validation errors so swallowed exceptions do not retain caller frame locals.

This builds on the earlier exception-cycle fix from #1295.
