test(clinvar): mock NCBI archive download in e2e job test

The cold download of the 2025-01 ClinVar archive (~350 MB) exceeded
the arq job_timeout=600 on CI runners, causing the e2e test to fail
with a TimeoutError on every push to main.

- Patch fetch_clinvar_variant_data with AsyncMock returning a minimal
  synthetic dict containing only allele 3045425
- The ClinGen API call (CA9765210 → 3045425) still runs live, keeping
  the happy path genuine end-to-end
- Drop @pytest.mark.slow since the test is no longer slow; schema/
  archive compatibility is covered by TestClinVarSchemaCompatibility
