fix(scripts): stop round-trip reporting unchanged and start-codon proteins as disagreements

A start-codon change has no predictable protein effect (p.?), which VRS cannot
express, so report it separately instead of as a failed re-derivation. Two
notations for an unchanged protein, such as hgvs's p.Ter330Ter for an
insertion that creates a stop right after the last residue and RT's
p.Thr328=, now count as a match.
