feat(plugins): enhance plugin system with agent loading and demo restructure

Plugin system improvements:
- Support namespace:agent format for loading agents (e.g., "yoker_plugin_demo:demo")
- Support plugin:// URLs for agent definitions
- Track plugin tools and agents in AgentCore
- Agent definition model override from agent files
- Enhanced demo plugin with pyproject.toml package structure
- Fixed namespace resolution for plugin tools and agents
- Added tool availability display in /tools command
- Enhanced /agents command to show plugin agents

Demo plugin restructure:
- Moved from flat structure to proper package (yoker_plugin_demo)
- Added pyproject.toml for installable demo
- Removed old __init__.py based structure
- Updated README with installation instructions

Testing improvements:
- Added comprehensive test coverage for agent loading
- Tests for plugin skill discovery from directories
- Tests for agent tool namespacing
- Test fixtures in conftest.py
- Command-specific test organization
