feat: color and group ai tokens output by provider (#23)

## Description

`arc ai tokens` now colors each row by its provider — matching `arc ai
usage` — and orders rows into per-provider bands by default so the
colors read as contiguous groups.

### Updated

- Color the token-history `group`/`provider`/`model` cells with the same
per-provider accents used by `arc ai usage`.
- Make the canonical table renderer ANSI-aware (measures and pads by
visible width) so colored cells stay aligned.
- Default `--sort-by` is now `cluster`: providers ordered by combined
cost, models by cost within each band; explicit `--sort-by cost` still
gives the flat sort.

### Added

- Tests for ANSI-aware table alignment and the new clustered sort
default/override.
