Add adaptive planning policy crate (#78)

* Add adaptive planning policy crate

- add the adaptive planning-policy crate with deterministic PCA-to-DCBC switching diagnostics and one-way switch behavior
- integrate the new adaptive built-in planning option into the streaming indexer for divisive and agglomerative flows
- add spec and validation coverage for the new adaptive crate and indexer integration

- fail explicitly when adaptive PCA diagnostics cannot supply retained cumulative variance
- require the first adaptive planning segment to run directional PCA before any switch
- expose adaptive decision records through the indexer and strengthen switch-path validation coverage
