fix(mind_map): review round — orphan reachability, cycle guards, strict mode, livebook polish

- branch_reachable now follows branch_successors only (ignores associates)
- depth/1, paths/1, max_width/1 threaded with visited set to avoid cycles
- max_width no longer double-counts multi-parent nodes
- node_data drops DecisionTree :feature vestige; label falls back to to_string(id)
- add MindMap.new(strict: true); branch/4 and associate/4 raise on missing nodes
- set_root/3 error now includes existing root id
- Viewable.rebuild/2 retypes promoted root node
- cyclic?/1 docstring clarified (checks all edges, not just branches)
- render/mermaid.ex: remove dup Theme helpers, document :mindmap limitations,
  add cycle guard to native mindmap renderer
- livebook: tabs (Mermaid first), hex/local convention, fix Elixir typo,
  fix stale collapse comment, soften DAG claim
