fix(decision_tree): review round — prune orphans, strict mode, livebook polish

- prune_redundant/1 now removes orphaned child nodes and their edge_meta
- add DecisionTree.new(strict: true); branch/4 raises on missing nodes
- set_root/3 error now includes existing root id
- Viewable.rebuild/2 retypes promoted root node
- remove duplicate Theme helpers header in render/mermaid.ex
- livebook: tabs (Mermaid first), hex/local convention, real inconsistent_paths demo,
  fixed PlantText hint, zoom level 0 demo, corrected orphan framing
