[#229] Add feature design documentation system for form builder

- Add FB-001 form builder data architecture design document
- Add reusable feature design template for future features
- Update CLAUDE.md to reference design documents location
- Document all key decisions: status field, versioning, schema storage
- Include API contract, type mappings, and mobile sync impact

This establishes a documentation standard for team members to follow
when planning new features with Claude.
