feat(elreal): converge on online arithmetic; faithful single-block twoDivZBCL (#1067)

Make the streaming infSum/mul/div the canonical LFPERA arithmetic, deprecate the eager scaffolding with a removal plan (docs/design/elreal-online-convergence.md), and fix the dense multi-block division cost explosion by following McCleeary's dissertation: twoDivZBCL returns the single-block remainder (Def 4.1.12, block_two_div_rem) and recurses single-block-by-single-block instead of fanning out. Validated subset (infsum==sum, mul==exact product, single-block/sparse/shallow-dense div) in el_arith_online_muldiv; full elreal suite 35/35 gcc+clang. Remaining host-floor handling for deep dense divisors tracked separately. Relates to #1061.
