testing.md: paginate coveralls source_files.json in coverage audit (#4428)

The audit snippet used `?per_page=1000` which returns only the first
page of files. MO's build now exceeds 1000 source files, and every
Phlex view (`app/views/**/*.rb`) lands on page 2 — so the snippet
silently reported them as `<not instrumented>`, a false negative.
Rewrite to walk pages until exhaustion, merge into one lookup, and
note in the rule that a `<not instrumented>` reading on a known-
tracked file should make the reader suspect pagination.
