docs: warn against passing raw exceptions as error message

str(e) on exceptions that embed per-entity payloads (tracebacks,
floating-point data) produces a unique message per entity, causing
one GitHub issue per occurrence instead of deduplicating. Update
examples to use type(e).__name__ and add an explicit callout.
