fix: graceful ConfigMap template fallback when RBAC permissions are missing (#12)

Template tools (list_templates, get_template, spawn_workspace) no longer
crash when the service account lacks configmaps permissions. They now
fall back to local/built-in templates with a warning.

- Extract findLocalTemplate to ~/k8s/local-templates.ts for reuse
- Unconditionally register list_templates and get_template tools
- Wrap listTemplateMaps in try/catch with warning in response text
- Add .catch() fallback to readTemplateMap in spawn_workspace
- Add ADR-010 documenting the design decision
- Bump version to 0.5.1
