ci: use npm pack instead of npm publish --dry-run

npm publish --dry-run fails when the version already exists on the
registry (npm 11 behaviour). npm pack validates the same package
structure and also produces the tarball needed by the vsce step.

Also fix the vsce package step: file:../.. creates a symlink that vsce
follows, pulling in the entire repo root. Override xq-lsp with the
packed tarball (npm install ../../xq-lsp-*.tgz) so vsce sees a real
directory instead of a symlink.
