fix: plugin hook correctness cluster (OU-172, OU-178, OU-179, OU-199) (#79)

four hook bugs bundled. all are reliability/UX fixes for the persist workflow.

- OU-172: PostToolUse matcher missed MultiEdit and NotebookEdit. Extended hooks.json matcher; post-edit-check.js now branches on tool_input.edits array for MultiEdit, falls through to file_path for the rest.
- OU-178: ALREADY_PERSISTED_PATTERNS suppressed legit nudges on natural language ("I'll persist this", "the knowledge base..."). Tightened 5 loose regexes down to 2 structured-confirmation anchors: [ouroboros] persisted N entries and mcp__*__put action payloads.
- OU-179: stop.js double-persisted blocks already written by /persist. /persist now emits _persisted_by: "persist-skill" sentinel in the fenced kb block; persistKbBlock skips blocks bearing the sentinel.
- OU-199: subagent-start.js queried recent-only KB and no items. Now does prompt-based KB search + 5 open items, two-section output, 60s file-based session-scoped cooldown. Dropped redundant double logHookEvent call.

plugin tests: 190 -> 211 (21 new). Go tests unchanged and green.
