refactor: register all MCP tools eagerly at startup (OU-164) (#77)

After OU-205 cut the MCP surface from 9 tools to 4, tier-1 deferral was no longer worth its complexity. The savings (~325 tk at startup) are small compared to the costs: subagents starting without write tools (OU-199), test scaffolding around tier1Once leaks (OU-187), and the "why isn't put available?" surprise.

- delete tier1Once, registerTier0, registerTier1, unlockTier1
- collapse buildServer to register all 4 tools (get, search, put, item) in one eager pass
- delete handleGetWithProgress, handleSearchWithProgress, handleItemWithProgress passthrough wrappers
- replace TestProgressiveRegistration with TestAllToolsRegisteredAtStartup
- regenerate tools_list.json snapshot to reflect 4 tools at startup (was 2)

closes OU-164. closes OU-187 (tier1Once leak resolved by deletion).
