docs: strip stale references to removed MCP tools (post OU-205) (#76)

PR #75 cut MCP tools from 9 to 4 but left plugin skills, agents, README, and CLAUDE.md still referencing the removed `project`, `plan`, `config`, `delete`, `export`, `import` tools.

- plugin/skills/recall: drop plan tool call and plans output bullet
- plugin/skills/triage: drop project/plan tool calls; items + KB only
- plugin/agents/backlog-manager: drop project/plan/config from frontmatter tools list; narrow scope to items; project-missing failures redirect user to CLI
- plugin/agents/knowledge-explorer: drop plan/project from frontmatter tools list; remove plans research step
- README.md: update tools table to 4 MCP tools; note CLI-only ops
- CLAUDE.md: same as README

subagent scope narrowed deliberately — plan/project ops are deliberate user actions invoked from main context, not subagent automation.
