* realtime: decode_market_depth_exchanges proto-only at floor 213 (PR-C4)

Gate is PROTOBUF_REST_MESSAGES_3 (213), the connection floor since #632.
Flip decode_market_depth_exchanges to require_proto() over &ResponseMessage;
drop server_version arg (legacy text branch swapped service-data-type
encoding pre-SERVICE_DATA_TYPE). Sync/async fixtures migrate from raw
"71|2|..." text frames to proto_response(IncomingMessages::MktDepthExchanges,
...encode_proto()).

Add MktDepthExchangesResponse + DepthMarketDataDescriptionFields builders
in src/testdata/builders/market_data.rs. Modernize the in-file
test_decode_market_depth_exchanges_proto unit test to use the builder
(rule 9). Delete test_decode_market_depth_exchanges_text_path and the
_old_version sibling - the dual-format text branches no longer exist - and
add test_decode_market_depth_exchanges_rejects_text_framing per the
proto-only convention.

Mark ResponseMessage::next_bool #[allow(dead_code)] (final-cleanup deletion
candidate; its only consumer was the deleted < SERVICE_DATA_TYPE branch
above. Matches PR-C2 treatment of next_long.
