test(cli): push cmd/ingitdb/commands coverage from 95.7% to 96.2%

- Remove unreachable default switch cases in delete.go, select.go,
  update_new.go (sqlflags.ResolveMode only returns ModeID/ModeFrom)
- Remove dead error check in describe.go (discoverCollectionChildren
  never sets its named err return)
- Add coverage_100_test.go with 20 tests targeting remaining testable
  paths: CSV write errors, editor ReadFile-after-delete, remote branch
  errors, batch rollback, docs-update conflict resolution, MCP handler
  error paths

Remaining 3.8% uncovered consists of:
- yaml.Marshal/json.MarshalIndent errors on well-formed types
- OS-level I/O errors with no injection seam
- Transaction error paths requiring mock dal.DB implementations
- Complex git-conflict integration scenarios in rebase/docs_update
