test(cli): increase cmd/ingitdb coverage from 54% to 92%

Extract launchTUI from runTUI to make the post-TTY-guard logic
testable. Add tests for all launchTUI error paths (resolve path,
readDefinition failure, nil definition) and the happy path with
cancelled context. Add explicit runTUI non-TTY guard test.
