Phlex conversion rule: watch for decorative Model.new passed to super (#4356)

Adds a note to `.claude/rules/phlex_conversions.md` under "Phlex Form
Conversions" describing the anti-pattern of passing a throwaway
`Foo.new` to a Superform's `super` *just* to satisfy the API while
hand-rolling every input with raw `name=` strings.

Drawn from the `OccurrenceResolveForm` → `OccurrenceProjectsForm`
refactor in #4345, which replaced `super(Occurrence.new, **)` with a
real `FormObject::OccurrenceProjects` that the form's fields
actually bind to.

Includes a "fuzzy cases" section acknowledging the gradient: real
forms sometimes sit between "this is an `X` form" and "this is an
operation that incidentally touches `X`", and a FormObject isn't
always the right answer.
