refactor(server): extract ServingCore for lock/step-down invariants (#394)

Bundle the allocator, serving-state channel, and both extension locks into
a private ServingCore. The extension read barrier is reachable only through
the guard that holds extension_lock, so the extension_lock -> extension_gate
order is now a compile-time guarantee; step_down's clear-before-publish and
gate-free transition, and the fence's write-only barrier, are private and
unit-tested. Server keeps consensus/clock/config and holds the core by value.
