fix: collect inline xmlns: namespace declarations from element constructors

extractNamespaceDecls only looked at prologue NamespaceDecl nodes, so a
prefix declared via xmlns:prefix="uri" directly on a DirElemConstructor
was treated as undeclared, causing spurious XQST0081 diagnostics.

Extend extractNamespaceDecls to also walk DirAttributeList nodes and
collect QName children whose value starts with "xmlns:". Add a helper
allTerminalValues to reassemble the URI from the character-by-character
DirAttributeValue sub-tree.

This eliminates the XQST0081 false-positives across fn-serialize,
prod-DirElemContent.namespace, prod-CompNamespaceConstructor,
app-FunctxFn, app-FunctxFunctx, and several other test suites.
