refactor(test): split diagnostics.test.ts into per-feature files

Replace the 1185-line monolithic diagnostics.test.ts with 9 focused
test files (one per source module) plus a shared test-utils.ts.

Use table-driven tests for repetitive diagnostic patterns:
- namespace-diagnostics: REPORTED/SUPPRESSED case tables
- functioncall-diagnostics: ARITY_ERRORS/NO_ERROR tables
- unused-diagnostics: FLWOR_WALKER_CASES table (6 structurally
  identical "scoped binding does not hide module variable" tests)

All 111 original test cases are preserved; 159 tests pass in total.
