Refactor new palette API to use token hooks (#2840)

The primary goes of this PR was to refactor the new API created for the palette module to use token hooks instead of providing it's own. This helps to keep token management central to the tokens module and prevents the need to import plugin modules at all except for initial bootstrapping.

As part of these changes, this PR also introduces some new themes to the documentation website. This also demonstrates how to create, manage and toggle modes as well as themes.

### Refactored palette variants

Palette variants are no longer output as CSS vars. Instead, to reduce CSS var output, only the base color and `lch` parts are output as CSS variables. Instead, palette variants are used as a look-up map for when a tokens get request is made and a variant key is provided.

**Additional changes**

- `core`: New default color palette!
