chore: bump to 3.0.0 and promote Node engines bump to breaking (#349)

The unreleased work raises engines.node from >=16 to >=18, which the
v2.0.0 entry already established as a breaking change for this project.
Reframe the changelog under 3.0.0 with a Breaking changes section and
align package.json.
