Fix Coding with QA workflow gates (#1973)

* fix: tighten coding QA workflow gates

Require browser QA evidence for UI changes, restrict gate writers, and keep PR readiness checks/worktree sync safe before Forge usage.

* fix: address QA workflow review feedback

Keep BLOCKED PRs routable before review, retain the reviewer writer alias, and align QA artifact instructions with required browser evidence.

* test: cover blocked PR gate readiness

Exercise the coding workflow gate with BLOCKED mergeStateStatus so required-review protection remains routable before review.

* fix: preserve coder gate writer alias

Allow renamed Coding workflow nodes to keep publishing PR URLs through the stable coder slot alias.

* fix: use PR host for review thread polling

Pass the parsed PR hostname to GitHub GraphQL polling so enterprise PR URLs query the correct host.
