Filter and transform Mycoportal report rows

Omit observations whose consensus name is in an exclusion list
(Duplicate, Undetermined, Mixed collection, Non-fungal, Slime-flux,
Eukarya/Eukaryota) or belongs to a non-fungal/non-Protozoa kingdom.

Reduce infrageneric names (Stirps–Subgenus), unpublished names
(nom. prov., comb. prov., nom. ined., etc.), and Gen. code names to
genus level: sciname becomes the genus, identificationQualifier becomes
"aff. <rank>", and taxonRemarks holds the search_name.

Add include_row? hook to BaseTable so subclasses can filter rows after
extend_data! populates extension values.
