vm: lower array spread entries (`[..., ...\$expr]`) natively

Extracts SpreadIntoArray helper from runArray.Run (the array/Iterator/
IteratorAggregate unpacking dance with PHP 8.1+ key handling), then
emits OP_ARRAY_SPREAD_APPEND for each spread entry. The in-progress
array stays on the VM stack between entries — spread pops just the
source value and merges into the array on TOS.

Bodies containing spread arrays are now slot-safe.
