Document style preferences and PR workflow in AGENTS.md (#2713)

Add what the existing AGENTS.md was missing:

- Code Style — encode the project's three control-flow rules
  (guard clauses over trailing if/else, no return inside case/when,
  no case/when for class-identity dispatch) and the comment policy.
- Commits and PRs — make explicit that branches keep one commit
  amended in place (no stacking review/CI commits) and that the
  push is always `--force-with-lease`. Add the split-by-concern
  principle.
- Changelog — short single-line entries; PR number filled in by
  amending after the PR is opened.
- UPGRADING.md — narrow the trigger from "breaking or behavior
  change" to "contract break", with examples of what doesn't qualify.
- Rebasing Open PRs — new section with the fetch/rebase/spec/push
  workflow and a note on CHANGELOG / lib conflict resolution.
