Fix broken PyPI links + bump to 4.22.1

PyPI renders README.md verbatim and resolves relative links against the
project page URL, so links like ./docs/effect_annotation.md became
pypi.org/project/varcode/4.22.0/docs/effect_annotation.md (404).

Rewrite the Further-reading section's relative links to absolute URLs:
- Doc guides point at the published openvax.github.io/varcode/ site.
- Quick-start notebook and CHANGELOG point at github.com blob URLs.
- docs/ index points at github.com/openvax/varcode/tree/main/docs.

PyPI doesn't allow re-uploading the same version, so this needs to ship
as 4.22.1 — 4.22.0's PyPI page will remain broken.
