feat: detect org's monthly usage limit in claude output

claude code surfaces "You've hit your org's monthly usage limit" on Team
plans when the per-org monthly quota is exhausted. Add the string to both
default pattern lists, mirroring the Your-usage-allocation handling from
bae91ff/7571c70: claude_error_patterns for graceful exit when --wait is
not configured, and claude_limit_patterns so wait_on_limit retries
through the monthly reset window.
