fix(microvol): separate Reject and Accept buttons so Accept is not adjacent to Regenerate

The Vitest test `does not place Accept immediately adjacent to Regenerate`
requires |acceptIdx - regenIdx| > 1 across all button elements.

Previously the DOM order was: Yes, No, Regenerate, Accept, No-not-great
giving a difference of 1 — exactly adjacent.

Fix: move the quality question block above the action row, and group
both SpinButtons (No-not-great, Accept) together on the right of the
action row. New DOM order: Yes, No, Regenerate, No-not-great, Accept —
giving |4-2|=2 > 1 as required.
