test(orders): builders + body-verifying test migration (#497)

* test(orders): builders + body-verifying test migration

Request builders for place/cancel/(all/auto)open/completed/executions/
global_cancel/next_valid_order_id/exercise_options. Simple-response
builders for OrderStatus, CommissionReport, ExecutionData, OpenOrderEnd,
ExecutionDataEnd, CompletedOrdersEnd. OpenOrder/CompletedOrder builders
deferred (~100+ fields; existing inline literals work).

Migrated sync/async tests: assert_proto_msg_id -> assert_request<B>
body verification; small response literals replaced with builders.
Three builder->production-decoder integration tests added in
orders/common/decoders/tests.rs.

Dropped 11 inline self-loop encoder tests per PR 2 lessons. Un-gated
MessageBusStub::with_responses from cfg(feature="sync") so async
tests can use it directly.

* docs(claude.md): set 90% line coverage target
