* docs: eliminate-mock-gateway coverage audit (PR 3/5)

Per-test disposition table for client/sync/tests.rs (60 tests) and
client/async/tests.rs (57 tests). Categorizes each as duplicate /
migrate / parity-fix per todos/eliminate-mock-gateway.md §3.

Findings:

- 109/117 tests are MockGateway-driven duplicates of per-domain
  MessageBusStub tests in <domain>/{sync,async}/tests.rs. Safe to
  delete in PR 5.
- 6 unique tests need migration to connection/{sync,async}_tests.rs in
  PR 4: test_connect (handshake) and test_disconnect_{completes,
  is_idempotent} on each side.
- 3 parity gaps require new tests in PR 4: exercise_options sync,
  subscription_cancel_only_sends_once async, client_id async.
- 2 stub-based tests are "in the wrong file" rather than coupled to
  MockGateway: test_client_id and test_subscription_cancel_only_
  sends_once. They move to per-domain or sibling files in PR 4/5.

Spot-checked 12 of ~109 duplicate rows by grep against per-domain
test files; pattern holds. Verification protocol for PR 5 is
documented at the bottom of the audit.

* docs: update master plan with PRs 1-3 status and concrete PR 4 scope

* docs: correct sync test count in audit summary (60 → 59)
