refactor(provider)!: rename CannonSingletonApplyError and guard double-dispose

Rename exported error to CannonSingletonApplyTwiceError for clarity and add a
static assert helper. Provider.dispose() now throws ProviderDisposedError when
called twice.

BREAKING CHANGE: CannonSingletonApplyError is renamed to CannonSingletonApplyTwiceError.
