v1.30.28: Raise class-II implausible cutoff from 40aa to 45aa (#217)

Per-user direction. v1.30.22 raised the class-II severity-tier
implausible cutoff from >=31 to >=40 to keep ~13K legitimate
Stražar 2023 mono-allelic HLA-II ligands in the 31-39aa range.
v1.30.28 bumps it again to >=45 -- the 40-44aa tail is sparse
but still genuine class-II biology (Stražar's published tail
extends to 51aa).

Test fixture: 40aa class-II is now "ok" (was implausible in
v1.30.22-v1.30.27); 45aa class-II is the new implausibility
boundary.
