refactor(webui): extract Diagnostics SSE state machine to lib/sse.ts (TA-310) (#285)

- new SseClient class (pure TS, no Svelte stores) owning EventSource lifecycle, exponential-backoff reconnect, and stall detection
- Diagnostics.svelte loses ~110 lines of inline state; visibility-change reconnect kept (DOM-coupled)
- 10 vitest cases with fake EventSource cover open/message/error/backoff/reset/external-refinement/retry-countdown/stall/reconnectNow/destroy/stop
