feat(webui): pool row redesign + Remove buttons + setting placeholders (#277)

each pool row is now a two-column flex: an info column with caption
("Primary"/"Fallback" + inline ACTIVE badge), a single endpoint line
combining host:port · worker · wallet, and a settings-line for
per-pool toggles; right-aligned actions cluster (Switch | Edit |
Remove). drops the leading numbering and the password mask — neither
carried real signal. mobile layout stacks the actions below the info.

Remove primary calls DELETE /api/pool/primary (promotes fallback);
Remove fallback calls DELETE /api/pool/fallback. handleRemove freezes
the displayed pool the same way handleSwitch does so the page doesn't
flicker as the firmware reshuffles slots.

settings-line carries two disabled-pending placeholders for TA-306
(extranonce.subscribe) and TA-307 (decode coinbase) so the layout
already shows where those toggles will land.
