fix: refresh reviewer prompts after restart (#1728)

* fix: refresh node agent prompts on rehydrate

Ensure restored workflow node sessions reapply the current slot prompt before resuming so reviewer instructions do not drift after daemon restarts.

* fix: preserve rehydrate fallback for missing workflows

Avoid failing node-agent rehydration when workflow metadata has been deleted; keep the persisted prompt and continue resuming the session.
