* Fix PR ready gate handoff validation

Allow the coder agent to write the PR URL gate field and ensure the ready check validates the supplied PR directly instead of rediscovering it through branch filters.

* Allow protected PR ready status

Treat GitHub's BLOCKED merge state as ready for review handoff when the PR is otherwise open and mergeable, avoiding a pre-review deadlock on protected branches.

* Authorize gate writers by workflow node

Include workflow node names in node-agent writer aliases so gate fields can keep using node-based writers while still matching the active agent session.
