[copilot] Report Copilot context usage from bridge (#1680)

Consume Copilot SDK session.usage_info events in the bridge stream
handler to provide accurate context/token reporting through the
Anthropic-compatible bridge, without adding provider-specific logic
to daemon context retrieval or UI components.

- Add context-usage module: snapshot Copilot usage_info fields
  (systemTokens, toolDefinitionsTokens, conversationTokens,
  currentTokens, tokenLimit) into bridge-facing usage snapshots
- Wire session.usage_info into streaming: final SSE message_delta
  carries real input_tokens from Copilot instead of resetting to 0
- Add /v1/messages/count_tokens endpoint: returns request-scoped
  input_tokens estimate plus latest Copilot context snapshot
- Add /v1/models endpoint: returns Anthropic-compatible model
  metadata from Copilot client.listModels() with fallback list
- Add updateInputTokens() to AnthropicStreamWriter so usage_info
  can update the writer mid-stream before the epilogue

All integration stays within the bridge boundary — no changes to
ContextFetcher, UI context components, or provider-agnostic daemon code.
