Test coverage: /api/changes endpoint ratings and user updates

Added comprehensive TDD test coverage for GET /api/changes endpoint:
1. TestChangesRatingCommentField - verifies comment field in ratings
2. TestChangesRatingReasonField - verifies reason field in ratings
3. TestChangesRatingAllFieldsPresent - validates all Rating struct fields
4. TestChangesUserLastUpdatedNullHandling - verifies *string scanning for NULL
5. TestChangesResponseStructure - validates complete response structure

Tests verify database schema mapping: user.lastupdated, ratings array with
all fields (id, rating, comment, reason, tn_rating_id), and proper HTTP
status codes (200, 400, 403).

These tests ensure the recent /changes endpoint fixes for NULL handling and
missing Rating fields are properly validated.
