Re-ticking a ⚠️ item should retry, not skip

The applyConsentedChanges filter excluded items in the ⚠️ state outright,
but the user explicitly re-ticking a failed row IS the retry signal —
the bot must run the apply again. Drop the !i.failed condition; keep
the !i.applied guard so completed items still don't loop.

markItemsApplied already clears ⚠️ on a successful retry and re-marks
on repeated failure, so the lifecycle is correct end-to-end.
