refactor: extract config-related functions to settings.go

Move the 9 functions handling CLI options to Settings conversion,
defaults-template derivation, CLI overrides, auto-auth fallback, and
config DB load/save out of main.go into a dedicated settings.go. Move
the corresponding tests to settings_test.go to keep one test file per
source. main.go drops from 1864 to 1347 lines, main_test.go from 2615
to 1187 lines. No functional change.
