docs(plan): drop profile fields, pin earliest-root transition default, note role reuse

Removes the out-of-scope profile-fields notes. Pins the transition-phase
default for resolving a Space IRI to a space ref as earliest root by
dct:created (first-root-wins, deterministic). States explicitly that a
single gen:SpaceMemberRole nanopub may be attached to multiple spaces
via separate gen:hasRole nanopubs, each gated and indexed independently.
